Player Registration by Mail

Mail in registrations must be received by CLL before January 16^th, 2010

To register a player by mail, you will need to download the registration packet from HERE.

Complete page 4 and 5, the player registration and medical release.
Photo-copy the following:
  • Original birth cirtificate of registering player. Hospital copies are not valid.
  • Valid Parent/Guardian Drivers License
  • Valid Car Registration
  • Recent Utility Bill

Mail the completed forms, copies of proof of residency and payment to:
